How normal is it for a character in Call of Cthulhu
to go indefinitely insane within the first three scary things in the adventure? Because I was running a game, and...

If you start the game already insane, then the scary things won’t have much effect. https://1d4chan.org/wiki/Old_Man_Henderson

Yes, Old Man Henderson. The patron saint of derailing the plot train and dragging it twenty miles across the desert and into the sea.
But seriously. I was running a short scenario, a published one at that. Character sees dead body - slightly spoopy. Character has mildly unpleasant nightmare. More extremely mild spoop. Character draws worst possible nightmare on second night, nearly crit-fails the san roll, and then rolls for ten points of sanity out of (then) 38 or so. Proceeds to herp his derp.

That sounds like a lot of being unlucky adding up.

The really funny thing is that the party gets to find an artists’ camp that has been sacked by the gla’aki undead with the artists dragged off, and the paintings left behind are of various dreams the characters may have had. Which is a second pass attack on sanity if they have had that dream. But...
The module expressly states that maximum sanity loss from one source rules apply. So, he sees the painting of Gla’aki rising from the dark waters and impaling , fails sanity roll *again*... and, nothing. Maximum nope had already been achieved. There was no more nope to be had. The nope card was maxxed out. The nope store had nope backordered. We’re sorry, Mario, but your nope is in another castle.
So I figured he just stood there blinking for a minute or two like Lou Costello after seeing Dracula. Counting on fingers. Making gestures to himself. Choking . Shaking head, saying “hmm”.
